What Do I Need to Submit?

Every IRB Submission will contain the documents listed below.  The forms are designed to be submitted electronically.  However, any pages that require a signature must be printed, signed and mailed to the IRB Administrator at Box 6926, or be signed electronically.

 

1.  Investigator Agreement (for all PIs and co-PIs)

2.  Application for Initial Review of Research Protocol

3.  Informed consent documents

4.  Any study advertisements (verbal or written)

5.  CV for all PIs & co-PIs

6.  Any study surveys or questionnaires

7.  Proof of completed human subjects protections training for all persons involved with the      research.
